Job summary

St. Charles Health System is seeking a Mental Health Practitioner Intake and Referral to assess patient needs for acute crisis treatment and determine appropriate levels of care.

The role includes counseling, assessments, diagnoses, and referrals to the proper level of care within a fast-paced behavioral health setting. You will act as a facilitator for multi-disciplinary care plans, coordinate pre-certifications, and support discharge planning with families and community resources while

Qualifications

• Required advanced degree in psychology, social work, or related behavioral health field.
• Current licensure or ability to obtain within 3 years.

Responsibilities

• Coordinate screening, admission, assessment, and referral for psychiatric inpatient beds.
• Determine level of care, conduct crisis assessment and safety planning.
• Provide outpatient referrals and coordinate post-discharge follow-up.
• Lead interdisciplinary care planning for complex behavioral health needs.
• Deliver individual, group and family therapies using evidence-based modalities.
