Job summary

St. Charles Health System is seeking a Mental Health Practitioner for Intake and Referral to assess patient needs for crisis treatment and coordinate the appropriate level of care within our behavioral health services.

The role involves conducting crisis assessments, safety planning, and facilitating referrals to inpatient beds or outpatient services. The practitioner collaborates with the care team and coordinates with families, insurers, and external providers to ensure continuity of care.

Qualifications

• One year of experience in behavioral health or related field.
• Ability to obtain state licensure or license-eligible status within required timeframe.
• Bilingual in Spanish and English is a plus and may be required in some roles.

Responsibilities

• Assess patient needs for crisis treatment and determine appropriate level of care.
• Coordinate screening, admission, assessment, and referral for psychiatric beds.
• Provide outpatient referrals and coordinate care with outside agencies/hospitals.
• Participate in development of treatment plans and safety planning.
• Collaborate with Interdisciplinary Care Team and communicate with patients, families, and insurers.
