Pathways Forward is seeking a dedicated Mental Health Case Manager to assess and coordinate care for individuals needing mental health services in the Harrisburg area. The role emphasizes ethical engagement, timely access to services, and collaboration with a supportive team.

The position offers a full-time schedule, standard weekday hours, and a comprehensive benefits package designed to support staff and clients alike.

Qualifications

• High school diploma with 12 semester credit hours in social sciences and two years of public/private human services experience, with one year of direct contact with mental health consumers.
• Or a high school diploma and five years of mental health direct care experience.
• Act 33/34 clearances.
• Access to a vehicle for work-related travel.

Responsabilités

• Interview prospective mental health consumers to determine eligibility and service needs.
• Arrange evaluations and explain services available through the agency.
• Coordinate referrals and authorizations for levels of case management.
• Monitor and document consumer progress and liaise with providers.
• Advocate for consumers and respond to emergencies with referrals.

Formation

Bachelor’s Degree in sociology, social welfare, psychology, or related field

High school diploma with 12 semester credit hours in social sciences

High school diploma with five years of mental health direct care experience
