Job summary

Pathways Forward is seeking a dedicated Mental Health Case Manager in Harrisburg to assess and coordinate care for consumers needing mental health services. The role emphasizes ethics, eligibility assessments, and timely service delivery within the County MH/MR Program.

Qualifications

• High school diploma with 12 sem credits in social sciences and two years in public/private human services with direct contact.
• Or a high school diploma with five years of mental health direct care experience.
• Act 33/34 clearances required.
• Access to a vehicle for work-related travel.

Responsibilities

• Interview prospective mental health consumers to assess eligibility for services.
• Coordinate and facilitate movement through the service delivery system to ensure timely care.
• Arrange evaluations and referrals to determine service needs.
• Maintain records and documentation of activities.
• Participate in staff meetings and ongoing training.

Education

High school diploma + 12 sem credits in social sciences

Bachelor's Degree in sociology or related field
