Qualifications

• Ability to work effectively with individuals from diverse backgrounds and with varying support needs.
• Valid driver's license and reliable transportation if required for community-based client services.

Responsibilities

• Identify client needs, strengths, barriers, and available support systems.
• Assist clients in accessing mental health, medical, social, and community services.
• Coordinate referrals and follow up to ensure services were initiated and needs addressed.
• Collaborate with clinical staff to support treatment and service planning.
• Assist clients with navigating housing, transportation, employment support, benefits, and other services.
• Monitor changes in client needs and escalate as needed to clinical personnel.
