Job summary

St. Luke’s University Health Network seeks a Mental Health Case Manager to partner with clients and families to facilitate mental health services, and, when applicable, intellectual disabilities/autism services and substance use recovery.

Services may occur at home, in the community, work, or school as clinically needed. This role sits within Penn Foundation departments including Mental Health Case Management, Early Intervention Services, Intellectual Disabilities Support Coordination, Victory

Qualifications

• Bachelor’s degree in human services or related field.
• Relevant coursework for case management and disabilities services.
• 2+ years in mental health, substance use, or veteran support preferred.

Responsibilities

• Provide outreach, monitoring, assessment, services planning, crisis intervention, support network building, mentoring, coaching, initial screenings, locate and coordinate needed services.
• Complete referrals and follow-up to all behavioral health supports identified, per policies.
• Coordinate with involved staff and community members; maintain documentation; participate in on-call as required.
