Qualifications

Responsibilities

• Conduct client intake and assess eligibility for emergency financial assistance.
• Develop individualized housing stabilization and self-sufficiency plans.
• Coordinate services and referrals with Wesley programs and wrap-around supports.
• Document all case management activities and progress in HMIS and required systems.
• Maintain confidential client files and timely reporting to meet funder requirements.
