Job summary

Terros Health seeks a dedicated Case Manager to join our Desert Cove Recovery Health Center interdisciplinary team in Phoenix. You will support individuals with serious mental illness or general mental health needs under Court Ordered Treatment, coordinating services and fostering recovery goals.

No prior experience required; a Bachelor's in behavioral health is required and we provide comprehensive training.

Qualifications

• Bachelor's degree in behavioral health required.
• No experience needed; training provided for new grads.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and individualized service plans.
• Collaborate with clinical team to coordinate care with clients, families, providers and resources.
• Conduct outreach and follow-up to ensure resources are available and in place.
• Perform all case management functions including assessment and service planning.
• Pursue best practice outcomes for mental health and housing stability.
