Job summary

Terros Health in Phoenix is seeking a Case Manager to join our interdisciplinary team serving individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at the Mitchell Recovery Health Center. This full-time role offers a 40-hour work week, Monday through Friday 8:00am–5:00pm.

You will assess, plan, coordinate, and advocate for clients, collaborating with clinicians, family, and community resources to deliver integrated behavioral and medical care.

Qualifications

• Bachelor’s degree in behavioral health; training provided.
• No prior experience required.
• New grads welcome.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans, and individualized service plans.
• Collaborate with the clinical team to coordinate care with clients, families, behavioral health and medical providers, and community resources.
• Conduct outreach and follow-up to ensure resources are available and in place.
• Perform all case management functions including assessments and planning; identify need for further evaluations.
• Pursue best-practice outcomes in education, employment, housing, and community tenure.
