Vision Towards Peace Counseling Services seeks a compassionate, organized Case Manager to coordinate services for clients with mental health needs, develop individualized treatment plans, and advocate for clients’ rights within the clinic and with external providers.

The role involves intake and assessment, care coordination, resource management, crisis intervention, documentation, and collaboration with clinicians. A Bachelor’s degree or equivalent is required, with 2+ years of experience.

Formación

• Bachelor's degree in a related field or equivalent experience.
• 2+ years of case management or similar mental health experience.
• Proof of education and years of experience required.
• Certification in Case Management (CCM) is a plus.
• CPR and First Aid certification is preferred but not required.

Responsabilidades

• Coordinate services and develop individualized treatment plans with clients and clinical staff.
• Conduct client intake and initial assessments.
• Assist with referrals to housing, employment, medical care, and legal services.
• Advocate for clients' rights and informed treatment choices.
• Maintain confidential case files and prepare reports.
• Coordinate holistic care with clinicians and other providers.
• Attend team meetings and participate in crisis intervention as needed.
• Provide ongoing support and follow-ups, including transitions in care.
