St. Luke's University Health Network seeks a Mental Health Case Manager to join our behavioral health teams. The role focuses on outreach, assessment, service planning, crisis intervention, and coordination with staff and community partners to ensure comprehensive care.

The position requires a bachelor’s degree in human services and relevant coursework, with on-going documentation and potential on-call duties as part of rotating shifts. A valid driver’s license is required.

Formación

• Bachelor’s in the human services field with relevant course work required for Mental Health Case Management, Early Intervention Services Recovery Center Inpatient and Intellectual Disabilities.

Responsabilidades

• Provide supportive services including outreach, monitoring, assessment, services planning, crisis intervention, support network building, mentoring, coaching, initial screenings, locate, coordinate and monitor needed services; in compliance with departmental policies and procedures.
• Complete referrals and follow-up to all behavioral health supports identified; in compliance with departmental policies and procedures.
• Ensure coordination and collaboration with all involved staff and applicable community members.
• Utilize de-escalation and crisis diversion skills and strategies to support participants who are in crisis.
• Maintain productively standards and complete all required documentation according to departmental guidelines.
• When applicable, participate in rotating on-call as required.
