Job summary

HELP of Southern Nevada is seeking a Healthy Living Intensive Case Manager in Las Vegas, NV to provide direct, intensive case management to chronically unhoused adults, facilitating transition to stable housing and self-sufficiency.

The role requires conducting intake, developing individualized plans, coordinating with services, and maintaining detailed case notes. Evening and field visits may be needed to support clients.

Qualifications

• Experience with intake and needs assessments.
• Strong client-centered case management skills.

Responsibilities

• Conduct initial interview/intake and client needs assessments.
• Provide case management and supportive services to move clients into long-term housing.
• Create individualized case plans for each adult household member.
• Review and update case plans monthly and track outcomes.
• Connect clients with wrap-around services and mainstream programs.
• Maintain HMIS daily and ensure audit-ready files.
