Job summary

HELP of Southern Nevada is seeking a Healthy Living Intensive Case Manager to support chronically unhoused adults in Las Vegas. You will conduct intakes, develop individualized case plans, coordinate housing and wrap-around services, and monitor client progress to promote stability and self-sufficiency.

Reporting to the AHP Supervisor, you will maintain HMIS records, attend case conferences, and partner with local agencies.

Qualifications

• Proof of Highschool Graduation or GED.
• Active Nevada Driver's License with clean MVR.
• 2 years’ experience with data entry, compliance, accounting or related field
• Previous experience working with unhoused population

Responsibilities

• Conduct intake interviews and assess client needs.
• Provide case management and connect to housing resources.
• Create individualized case plans and track outcomes.
• Coordinate wrap-around services with partner programs.
• Enter case notes daily into HMIS.
• Attend monthly case conferencing meetings.
• Maintain audit-ready files and professional boundaries.
• Facilitate Life Skills Training on-site 2–3 times per quarter.
• Adapt to shifting priorities and client caseload.
