Job summary

CarePlus NJ, Inc. in Paramus, NJ seeks a Part-time Case Manager to support adults with serious mental illness through integrated care coordination.

The role is on-site, 20 hours per week, with CBT/MI-based interventions and on-site and outreach services. Responsibilities include psycho-educational groups, intake assessment, care planning, collaboration with the treatment team, and linking to community resources.

Qualifications

• Bachelor's degree in a related field.
• Two (2) years of experience in a health care or behavioral health setting.
• A valid driver's license and must be insurable by the agency's automobile insurance.

Responsibilities

• Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness.
• Offer on-site as well as outreach services (including transport of consumers) and fulfill case management services.
• Ensure consumers have community support in place that fulfill a positive corroborative role.
• Communicate observations and information about the consumer in the consumer's service record and to team members.
• Participates in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge.
• Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans.
• Implement evidence-based interventions (such as CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter-agency coordination of treatment.
• Participate in quality assurance activities and cross-functional action teams
