Job summary

Greenbrook Mental Wellness Centers is seeking a Case Manager to serve as the primary contact for patients throughout their treatment journey in a clinic-based setting in MI. You will coordinate care, manage scheduling, and support insurance and prior authorization processes to help patients access treatment.

Ideal candidates demonstrate empathy, strong organization, and the ability to work collaboratively across teams in a fast-paced environment.

Qualifications

• Bachelor's degree and at least one year of patient care coordination or case management experience.
• Experience with EHR and CRM systems (AdvancedMD, Salesforce) preferred.
• Proficiency with Microsoft Office (Outlook, Word, Excel) is valued.

Responsibilities

• Coordinate care, manage scheduling, and support insurance and authorization processes.
• Guide patients from consultation through treatment completion for a smooth experience.
• Coordinate appointments, provider schedules, and treatment timelines.
• Maintain accurate patient records and document in CRM/EHR systems.
