Job summary

Greenbrook is seeking a compassionate Case Manager to serve as the primary contact for patients throughout their treatment journey. This clinic-based role coordinates care, schedules, supports insurance and prior authorizations, and helps patients overcome barriers to treatment.

You will collaborate with Authorization Specialists, providers, clinical teams, and revenue cycle partners to keep treatment plans on track, monitor progress, and maintain accurate records in CRM and EHR systems while

Qualifications

• Bachelor's degree and at least one year of experience in patient care coordination, healthcare operations, or a specialty care setting.
• Experience with EHR and CRM systems (AdvancedMD and Salesforce preferred).
• Knowledge of insurance benefits, prior authorizations, or healthcare scheduling is valued.

Responsibilities

• Guiding patients from consultation through treatment completion, ensuring a smooth and supportive experience.
• Coordinating appointments, provider schedules, and treatment timelines to maximize patient access and clinic efficiency.
• Monitoring patient progress and proactively addressing barriers that could delay care.
• Collaborating with Authorization Specialists, providers, clinical teams, and revenue cycle partners to keep treatment plans on track.
• Supporting prior authorization workflows and ensuring patients have appropriate coverage throughout treatment.
• Coordinating specialty pharmacy activities, medication access, and treatment readiness requirements.
• Maintaining accurate patient records, financial estimates, and documentation within CRM and EHR systems.
• Responding to patient questions with empathy, professionalism, and urgency.
• Helping ensure a high-quality patient experience while meeting operational and compliance standards.
