Job summary

Greenbrook Mental Wellness Centers is seeking a Clinic-Based Case Manager in Glen Allen, VA to be the patient’s primary partner throughout treatment. You will coordinate care, support insurance and authorization processes, and ensure a seamless patient experience, with a focus on mental health outcomes.

The role emphasizes organization, empathy, and collaboration with cross-functional teams in a fast-paced environment.

Qualifications

• Required: High school diploma or equivalent plus 3–5 years of experience in clinical case management or related healthcare setting.
• Experience using EHR systems and Microsoft Office applications.
• Strong communication, organizational, and customer service skills with ability to manage priorities.

Responsibilities

• Guide patients through their treatment journey from initial consultation to completion.
• Coordinate appointments, prior authorizations, scheduling, and pharmacy activities.
• Monitor progress, address barriers to care, and collaborate with cross‑functional teams.
• Maintain accurate patient records and documentation within CRM and EHR systems.
• Respond to patient inquiries with empathy and urgency while ensuring a high‑quality patient experience.
