Job summary

St. Luke’s University Health Network seeks a Behavioral Health Care Manager to support and coordinate mental and physical health care for patients on an assigned caseload.

This role works with the patient’s medical provider, psychiatric consultant, and broader care team to deliver brief, evidence-based interventions and ensure engagement in care. The ideal candidate holds a Master’s degree in social work or related field, with experience in screening for mental health and substance use disorders

Qualifications

• Master’s degree in social work, counseling, or related field.

Responsibilities

• Screen and assess patients for common mental health and substance use disorders.
• Coordinate care with medical and mental health providers as appropriate.
• Provide education to patients, families, and care teams about mental health disorders and treatment options.
• Track treatment response and monitor changes in symptoms; communicate with the care team.
• Support treatment adherence and coordinate referrals to specialty care.
• Provide brief evidence-based interventions (e.g., motivational interviewing, behavioral activation).
• Participate in caseload consultations with the psychiatric consultant and relay recommendations to the medical provider.
• Facilitate treatment plan changes in collaboration with the medical provider and psychiatric consultant.
• Maintain documentation in EPIC EHR and related systems; track registry data.
