Horizon Health is seeking a dedicated Care Coordinator in the United States. This remote, full-time position offers a competitive pay range of $22.10 - $25.00 per hour, depending on experience. You will provide care coordination for adults dealing with serious mental illnesses, manage assessments, and develop personalized care plans.

Formación

• 2 years of experience providing direct services to individuals with serious mental health issues.
• Clean, valid NYS driver’s license and reliable transportation required.

Responsabilidades

• Provide independent, person-centered care coordination for adults with serious mental illness.
• Conduct member assessments and develop care plans.
• Manage a caseload of approximately 40 members.

Educación

Bachelor's degree in a human service related field

Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
