Job summary

Boston Health Care for the Homeless Program (BHCHP) seeks a Community Access Outreach Specialist / Men’s Health Coordinator to deliver trauma-informed outreach, care navigation, and linkage services for individuals experiencing homelessness and substance use. The role focuses on engaging men, reducing health risks, and connecting clients with medical and social services.

Qualifications

• Associate or bachelor’s degree in a related field, or equivalent experience (minimum two years in human services).
• Experience working with individuals with substance use disorder required.
• Experience working with individuals experiencing homelessness, incarceration, or violence preferred.
• Commitment to culturally responsive, equitable care across diverse populations.
• Strong communication, organizational, and interpersonal skills.
• Lived experience is welcomed and valued.

Responsibilities

• Conduct street outreach to individuals experiencing homelessness and/or substance use, particularly in the Massachusetts Avenue and Melnea Cass Boulevard area.
• Provide trauma-informed outreach, care navigation, and linkage services within a multidisciplinary team.
• Connect individuals to primary care, behavioral health services, substance use treatment, housing resources, benefits, and other social services.
• Drive CCiR van operations and manage outreach supplies; document activities in Epic.
