Job summary

Sycamore Heights Health and Rehabilitation is seeking a compassionate Memory Care CNA to join our dedicated unit. You will provide hands-on care, assist with daily living activities, and support residents living with Alzheimer’s and memory-related conditions in a structured, safe environment.

Working with a collaborative team, you will monitor residents, report changes to nurses, and help maintain dignity and independence through patient, respectful interactions.

Qualifications

• Current CNA license in the state.
• At least one year of LPN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Provide hands-on care to residents, including assistance with ADLs such as bathing, dressing, grooming, and toileting.
• Support residents with memory-related conditions using patience, redirection, and reassurance techniques.
• Assist with mobility, transfers, and ambulation to ensure resident safety.
• Monitor and report changes in condition, behavior, or mood to the nurse.
• Obtain and record vital signs (blood pressure, pulse, temperature, and respiratory rate) as directed.
• Assist with meal service and feeding, while encouraging independence when possible.
• Maintain a safe, calm, and structured environment for residents.
• Provide emotional support and companionship, building meaningful relationships with residents.
• Assist with incontinence care and maintain proper hygiene.
• Ensure accurate charting and documentation of care provided.
• Maintain confidentiality of resident information at all times.
• Communicate effectively with nurses, team members, residents, and families.
• Follow the direction from licensed nursing staff and ask questions when clarification is needed.
• Support a positive team environment and serve as a role model for peers.
• Provide excellent customer service to residents, families, and visitors.
• Adhere to all facility policies, procedures, and infection control guidelines.

Education

Current CNA license in the state

Experience with MatrixCare (preferred)
