We are excited to welcome a compassionate, patient, and dedicated Certified Nursing Assistant (CNA) to join our Memory Care Unit team! This is a unique opportunity to make a meaningful difference in the lives of residents living with Alzheimer’s and other memory-related conditions by providing specialized, person-centered care in a supportive and nurturing environment.

As a Memory Care CNA, you will play a vital role in creating a safe, structured, and comforting atmosphere where residents can thrive. You will provide hands-on support with daily activities while building meaningful relationships, offering reassurance, and using gentle redirection techniques to support residents’ emotional and physical well-being. Your presence and care will help promote dignity, independence, and quality of life for those we serve.

Responsibilities:
  • Provide hands-on care to residents, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting

  • Support residents with memory-related conditions using patience, redirection, and reassurance techniques

  • Assist with mobility, transfers, and ambulation to ensure resident safety

  • Monitor and report changes in condition, behavior, or mood to the nurse

  • Obtain and record vital signs (blood pressure, pulse, temperature, and respiratory rate) as directed

  • Assist with meal service and feeding, while encouraging independence when possible

  • Maintain a safe, calm, and structured environment for residents

  • Provide emotional support and companionship, building meaningful relationships with residents

  • Assist with incontinence care and maintain proper hygiene

  • Ensure accurate charting and documentation of care provided

  • Maintain confidentiality of resident information at all times

  • Communicate effectively with nurses, team members, residents, and families

  • Follow the direction from licensed nursing staff and ask questions when clarification is needed

  • Support a positive team environment and serve as a role model for peers

  • Provide excellent customer service to residents, families, and visitors

  • Adhere to all facility policies, procedures, and infection control guidelines

Qualifications:
  • Current CNA license in the state.

  • At least one year of LPN experience in a long-term care setting is preferred.

  • Knowledge of MatrixCare preferred.
