Qualifications

• High school diploma or GED required; relevant training is a plus.
• Three to six months related care experience preferred.
• Knowledge of dementia and ability to engage residents with person-centered care.

Responsibilities

• Provides dementia care including adapted ADLs and meaningful engagement.
• Assists residents with daily living activities and encourages independence.
• Maintains residents' records and reports changes in health or behavior to supervisor.
• Assists with meal service and observation of resident nutrition.
• Initiates and participates in Daily Path engagement programs and activities.
• Fosters a safe, clean, and respectful living environment and collaborates with the care team.
