Job summary

Applicants should have a high school diploma or GED and some direct-care experience with older adults; additional training is provided to ensure quality care and resident well-being in a vibrant Brookdale community.

Qualifications

• High school diploma or GED and 3–6 months related experience preferred and/or training in direct service with older adults living with dementia.

Responsibilities

• Assist residents with ADLs and daily living activities per the individual service plan.
• Document changes in resident health and behavior; report to supervisor.
• Serve meals and encourage proper diet and fluids, respecting resident choice.
• Initiate or lead at least one Daily Path engagement program per resident life story or calendar.
• Maintain residents' records daily, including medications as applicable.
• Follow medication policies and state regulations for medication assistance where permitted.
• Maintain a clean, safe, and orderly environment for residents.
• Respond promptly to emergencies and resident requests for assistance; observe and report changes to supervisor.
• Encourage teamwork and cooperative interactions with co-workers.
