Responsibilities include meal assistance, documentation, and safe medication support as allowed by state regulations. The position emphasizes teamwork, resident-centered care, and adherence to safety policies across shifts.

Qualifications

• High school diploma or GED; 3–6 months related experience preferred.
• Knowledge of dementia, especially Alzheimer's type.
• Follow state regulations regarding medication assistance where allowed.

Responsibilities

• Assist residents with daily living activities per care plan.
• Know residents Life Story, preferences, and routines.
• Serve meals and support dietary needs and hydration.
• Maintain resident records daily and report changes to supervisor.
• Follow medication procedures as permitted by state regulations.
• Maintain a clean, safe, and orderly environment for residents.
• Encourage independence and engagement through Daily Path programs.
• Participate in team meetings and communicate resident needs.
