Job summary

The Social Worker in the Memory Disorders Clinic provides specialized clinical counseling and care coordination for patients with cognitive impairment and their families, while also serving as a Patient Navigator to guide through the healthcare system.

Working with an interdisciplinary team, the role supports education, advocacy, and access to community resources, and ensures follow-up across the care continuum.

Qualifications

• MSW required with clinical social work training.
• Eligible for Massachusetts LCSW licensure.
• Experience with geriatric or cognitively impaired populations preferred.

Responsibilities

• Provide counseling and emotional support to patients and families.
• Coordinate care, appointments, and medical records among providers.
• Educate on diagnoses, treatments, and insurance navigation.
• Advocate for patient rights and barrier removal to access care.
