Qualifications

Responsibilities

• Provide specialized counseling and emotional support to patients with cognitive impairments and their families.
• Coordinate advanced care planning and connect patients to community resources.
• Participate in interdisciplinary case conferences and address psychosocial issues.
• Educate patients and caregivers about diagnoses, treatment plans, medications, and health insurance navigation.
• Monitor patient progress and adapt support to care plans.
