Qualifications

Responsibilities

• Facilitate and support management of symptoms and problems with cognitive disorders.
• Manage CMS registry for amyloid-targeted therapies.
• Act as a liaison between the patient, infusion center, and memory care clinic.
• Triage patients by phone for refills, test results, and questions, obtaining physician direction as needed.
• Assist in enrolling patients in patient assistance programs for Alzheimer’s disease therapies.
• Coordinate multi-disciplinary care and prior authorizations for medications and treatments.
• Advocate for access to services on behalf of the patient.
• Ensure safety during amyloid-targeted therapies with follow-up calls and timely physician communication.
• Communicate test results and treatment plan instructions to patients per physician instructions.
• Work with the research team to coordinate appointments and care.
• Meet each patient at first appointment to explain role on memory care team.
• Schedule necessary appointments in a timely fashion.
• Assist office staff with prior authorizations and FMLA paperwork.
