Job summary

Hoag Memorial Hospital Presbyterian in Newport Beach, CA, is seeking a Clinical Nurse Navigator I for Memory & Cognitive Disorders to coordinate care across the continuum for patients with complex diagnoses. You will advocate for patients, liaise with physicians, and guide families from diagnosis through discharge or end of life.

Strong focus on education, patient experience, and resource navigation. Minimum 5 years nursing with 3 years in a related specialty is required.

Qualifications

• Minimum of 5 years’ nursing experience and 3 years in area of specialty.
• BSN required in several departments; Spanish language skills preferred.
• Certifications referenced include CRRN, BLS, ACLS within 30 days of hire.

Responsibilities

• Coordinate care across the continuum for patients with complex diagnoses, serving as a patient advocate.
• Educate patient and family on health care options and coordinate services and resources.
• Contribute to quality improvement by gathering and analyzing quality metrics and outcomes.
