Job summary

The Blair Memory Care Center of Cary is seeking a compassionate Special Care Coordinator to lead Memory Care services, focusing on individualized care plans, staff training, and resident engagement. This role partners with the Executive Director, healthcare teams, and families to ensure a safe, enriching environment for residents with cognitive impairments.

Qualifications include 2+ years in senior living or healthcare, RN/LPN license preferred, and strong leadership and communication skills.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Care Plan Development & Management: develop and evaluate memory care programs and care plans reflecting residents' history and preferences.
• Staff Supervision & Training: provide leadership and dementia training; manage staffing levels.
• Resident & Family Engagement: serve as main contact for concerns; facilitate family meetings.
• Regulatory Compliance & Quality Assurance: ensure compliance with regulations and best practices; participate in quality audits.
• Collaboration: work with nursing, physicians, therapists, and leadership to coordinate care.
