Job summary

The Blair Memory Care Center of Cary seeks a compassionate Special Care Coordinator to oversee memory care programming and care coordination for residents, partnering with the Executive Director and care teams to ensure individualized, dignified care.

You will develop care plans, supervise staff, engage residents and families, ensure regulatory compliance, and collaborate with nursing and therapy teams to maintain high standards of quality and resident satisfaction.

Qualifications

• Bachelor’s or Associate degree in Nursing, Healthcare Administration, or related field.
• LPN or RN license preferred (state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Lead development, implementation, and evaluation of memory care programs promoting cognitive function, social engagement, and emotional well-being.
• Provide leadership, coaching, and memory care training to staff; ensure adequate staffing levels for high-quality care.
• Serve as primary contact for residents and families; facilitate meetings and communicate updates compassionately.
• Ensure memory care program compliance with state regulations, policies, and best practices; participate in quality audits.
• Collaborate with nursing, physicians, therapists, and leadership to coordinate care and support operations.
