Job summary

Sterling Healthcare Management is seeking a Memory Care Program Coordinator for Elwood Health & Rehabilitation. You will lead the memory care team, oversee care plans, and work closely with physicians to ensure exceptional dementia care and resident well‑being.

Ideal candidates will have a BSW (or equivalent), Alzheimer's certification preferred, and strong supervisory and English communication skills to support residents and families in a compassionate, compliant facility.

Qualifications

• Must graduate of an accredited school with a BSW, recreational therapist or equivalent
• Must have working knowledge of Alzheimer’s and Dementia
• Supervisory experience in healthcare or long-term care facilities preferred
• Must read, write and speak English clearly
• Knowledge of CMS regulations preferred
• Social Services designation preferred

Responsibilities

• Supervise staff and assign duties across shifts
• Monitor and update each resident’s Care Plan every 30–90 days
• Make daily rounds to ensure care plans are followed and resident status is documented
• Coordinate with physicians and record visits
• Ensure infection control and universal precautions are maintained
• Arrange transportation for residents to medical appointments
• Prepare incident reports and forward to ADON
• Provide compassionate, person-centered dementia care and leadership
