Job summary

Sterling Healthcare Management, LLC is seeking a Memory Care Program Coordinator for Elwood Health & Rehabilitation. The role focuses on leading memory care services, supervising staff, and ensuring high-quality, person-centered dementia care.

You will work closely with the care team to implement care plans, collaborate with physicians, and engage families to support resident well-being and safety.

Qualifications

  • Requires Alzheimer’s Certificate or willingness to obtain.
  • Strong knowledge of Alzheimer’s and Dementia care.
  • Supervisory experience in health care/long-term care preferred.
  • Must read, write, and speak English clearly.
  • Thorough knowledge of CMS regulations and patient care standards.
  • Demonstrates leadership, organization, and interpersonal skills.
  • Compassionate, patient-centered approach to residents and families.

Responsibilities

  • Oversee memory care unit operations and staff scheduling.
  • Supervise staff performance and ensure care plans are followed.
  • Monitor resident status and update care plans every 30–90 days.
  • Coordinate with physicians and document visits in chart.
  • Communicate with families about resident wellbeing and changes.
  • Ensure infection control and safety protocols are followed.
  • Collaborate with care team to deliver exceptional dementia care.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required for success as a Memory Care Program Coordinator.

  • Has or is willing to obtain Alzheimer’s Certificate
  • Must be a graduate of an accredited school with a BSW, recreational therapist or equivalent.
  • Must have working knowledge of Alzheimer’s and Dementia.
  • Qualified to assume the position by training and/or previous experience of a supervisory status, preferably in a health care and long-term care facility.
  • Must possess the ability to make independent decisions and mature judgments.
  • Must have the ability and the willingness to supervise and work harmoniously with professional and non-professional personnel.
  • Must be able to read, write, and speak English in an understandable manner.
  • Must be physically and mentally capable of performing routine job duties.
  • Excellence in all aspects of quality dementia care including exceptional person-centered care.
  • Good managerial, organizational, planning, and interpersonal skills.
  • Leadership qualities that motivate and direct the memory care staff.
  • Be personable with residents, families, and staff in a professional and cooperative manner.
  • Must have compassion, tolerance, and understanding for the elderly.
  • Thorough knowledge of CMS regulations.
  • Social Services designation preferred.
ESSENTIAL FUNCTIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.

  • Support the community’s philosophy of care and strive to achieve its goals and objectives.
  • Be sensitive to residents' families and respond in an appropriate professional way as the situation requires.
  • Assign all staff to residents--spelling out break times and all special duties to be completed.
  • Supervise all staff in performing their duties by checking their work closely to ascertain that assignments have been completed.
  • Make daily rounds on the wing to ensure that individual Care Plans are being followed and assess each resident's status in accord with his/her Care Plan.
  • Assist in updating the Care Plan every 30-90 days depending on the resident's condition and level of care at the Care Plan meeting.
  • Make rounds with physicians and record visits in chart.
  • Be responsible for well-being of all residents assigned to the unit while on duty. Contact families regarding significant decline in resident's status or transfer to hospital.
  • Ensure that nursing personnel provide treatments, diets, rehabilitative nursing care, activities of daily living, protective services, and training in self-care as stated in the care plan to ascertain that optimum comfort and care is provided.
  • Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care.
  • Make transportation and other arrangements for patients that have to see their physician or other outside professional medical provider.
  • Prepare incident and accident reports and forward them to the ADON.
  • Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility.
  • Assure that established infection control and universal precaution practices are maintained when performing care.
