DC West Linn Owner LLC in West Linn, OR seeks a Care Partner to support Memory Care residents with daily living activities and cognitive engagement. You will provide personalized support, assist with meals, grooming, and documentation, while maintaining a safe and calm environment.

The role emphasizes compassionate care, effective communication, and adherence to care plans, with opportunities to collaborate with the Health Services Director and Life Enrichment team.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving or healthcare settings preferred.
• Strong communication with residents and staff; ability to document observations accurately.

Responsibilities

• Provide personalized assistance tailored to memory impairments and stimulate cognitive function.
• Report incidents and participate in post-incident activities; monitor residents’ health changes.
• Assist with grooming, meals, and verify resident identity before serving; monitor dining room.
• Maintain resident documentation and charting; add residents to Alert Charting as needed.
• Maintain safe, structured environment to minimize confusion and promote independence.
• Engage residents in social activities and provide companionship; assist Life Enrichment Director as needed.
• Assist with personal care, mobility, toileting, meals, housekeeping, and medication per care plan.
• Complete all assigned duties and adapt to changes in resident needs and staffing.
• Report hazardous conditions and equipment to Health Services Director or Administrator; communicate concerns.
• Become familiar with Montessori Approach when applicable; keep supply area clean.
• No supervisory responsibilities.
