Job summary

Full Bloom Memory Care® in Lake Forest, IL is seeking a Memory Care Partner to provide activity-based dementia care and compassionate support. You will help clients stay active, engaged, and connected through conversation, creative activities, and community outings.

Qualifications

• Six months of dementia care or related caregiving experience preferred.
• High school diploma or GED required.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Ability to pass a background check and verify eligibility to work in the U.S.
• Ability to lift up to 50 pounds.
• Eligibility to register with IDPH Health Care Worker Registry.

Responsibilities

• Lead activities such as games, puzzles, music, baking, gardening, and crafts.
• Encourage movement through walks and healthy daily routines.
• Accompany clients to senior centers, parks, and community outings.
• Provide companionship and emotional support.
• Support independence while assisting with daily routines and personal care as needed.
