Full Bloom Memory Care in Lake Forest, IL is seeking a Memory Care Partner dedicated to activity-based dementia care. You’ll tailor daily plans to each client, lead engaging activities, and support mobility and independence while offering companionship and emotional support.

The role emphasizes relationship-building with clients and families and collaboration with the care team to ensure continuity of care in a compassionate, person-centered setting.

Qualifications

• Six months of dementia care, caregiving, education, behavioral health, recreation, or a related field preferred; professional or family experience welcome
• Reliable weekly schedule availability (e.g., Tue–Sat or Sun–Thu)
• Ability to pass a background check and verify eligibility to work in the U.S.
• Ability to lift up to 50 pounds
• Valid driver's license, reliable vehicle, and current auto insurance
• Eligibility to register with IDPH Health Care Worker Registry

Responsibilities

• Lead activities such as games, puzzles, music, baking, gardening, and crafts
• Encourage movement through walks and daily routines
• Accompany clients to senior centers, parks, and community outings
• Provide companionship and emotional support
• Support independence while assisting with daily routines and personal care
• Build relationships with clients and families
• Communicate with the dementia care team to support continuity of care
