Renewal Memory Partners in Chicago is seeking compassionate Care Partners to provide in-home support for individuals living with Alzheimer's and related dementias. You will offer companionship, assist with daily activities, and help maintain engaging routines in a person-centered approach.

Qualifications

• Compassion and patient nature are essential.
• High school diploma required; college degree preferred.
• Experience with Dementia/Alzheimer's is valued.

Responsibilities

• Providing proactive companionship to older adults and people with chronic health conditions in their homes.
• Planning activities outside the home and accompanying clients to keep them engaged.
• Escort clients to doctors' appointments.
• Running errands for clients.
• Reminding clients to take their medications.
• Doing light exercise with clients.
• Assisting clients in preparing meals.
• Performing light housekeeping, such as dusting, tidying, and laundry.
