Job summary

High Point Residence Kankakee is seeking a compassionate RN or LPN to join our supportive living and memory care nursing team. You will provide resident-centered care, work closely with residents, families, physicians, and the care team to support health and well-being in a memory care community.

You will conduct health assessments, administer medications, document progress, and collaborate to meet resident care goals while maintaining a safe, respectful environment.

Qualifications

• Current RN or LPN license in the State of Illinois.
• Current Basic Life Support (BLS) certification.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and organizational skills.
• Compassionate and resident-focused approach to care.

Responsibilities

• Conduct resident health assessments and monitor changes in condition.
• Administer medications and treatments as prescribed.
• Monitor residents and promptly communicate changes or concerns.
• Document resident care, progress, and responses to interventions.
• Collaborate with physicians, families, and team members to support resident care goals.
• Provide compassionate support to residents and families.
