Wind Crest by Erickson Senior Living is seeking a Memory Care Manager to lead a dedicated neighborhood for 36 residents, ensuring high-quality memory care and resident experience. You will oversee daily operations, support dementia best practices, and collaborate with interdisciplinary teams to create a safe, engaging home.

Qualifications

• Minimum of 3 years related experience in long term care/assisted living.
• Minimum of 3 years related experience in providing services for residents with cognitive impairment.
• Current CPR certification preferred.
• Knowledge of the geriatric population and the aging process.
• Demonstration of progressive approaches to supporting and enhancing the quality of life for seniors with cognitive impairment.

Responsibilities

• Conduct pre-residency assessments and screening of potential residents.
• Evaluating the psychosocial and ADL needs of residents in Memory Care and coordinating care and services.
• Monitoring the engagement of activities for residents with cognitive impairment, and participating in program development and planning.
• Leading a holistic, Person-Centered approach to assessment and care planning.
• Partner with other departments to develop support groups and educational programming for residents and families.
• Supporting Independent Living and Post-Acute Social Workers to transition residents between levels of care.
• Maintaining knowledge of Federal, State, and local regulations and facility policies regarding Memory Care.
