Job summary

National Church Residences in Columbus, OH is seeking a Part-Time LPN to provide memory care nursing from 7 pm to 11 pm. You will deliver direct care, implement the plan of care, and document accurately per policy.

Work with an interdisciplinary team, educate residents and families, mentor direct care staff, and participate in on-call rotations to ensure quality, respectful care for residents. A valid LPN license and ability to drive are required.

Qualifications

• Must have a current LPN license in good standing in the state(s) of practice.
• 12 months facility/agency nursing experience preferred.
• Ability to communicate in English; documentation and compliance training required.

Responsibilities

• Provides direct patient care as defined by the State Nurse Practice Act and physician orders.
• Implements plan of care initiated by the RN.
• Completes timely documentation per plan of care.
• Educates patients and families about diagnosis and care plan.
• Participates in coordination of home health services and reports needs to RN/Director.
• Mentors, coaches, and educates direct care staff under RN supervision.
• Uses equipment and supplies effectively and participates in on-call rotations.
• Participates in interdisciplinary team meetings to coordinate care.
