Job summary

National Church Residences is seeking a Part-Time Licensed Practical Nurse for Memory Care, 7p-11p shifts. You will provide nursing care, implement the plan of care, and document accurately under the guidance of the RN and Clinical Leader.

Responsibilities include patient and family education, coordinating home health services, mentoring staff, and participating in on-call rotations and interdisciplinary team meetings.

Qualifications

• Requires a nursing degree and current LPN license.
• Must have 12 months of facility/agency nursing experience.
• English language proficiency required.
• Must have a valid driver's license and auto insurance.

Responsibilities

• Provides direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family education pertinent to diagnosis and plan of care.
• Participates in coordination of home health services and reports needs to the RN/Case Manager.
• Mentors, coaching, and education to direct care staff under RN supervision.
• Participates in on-call rotation and interdisciplinary team meetings.
