Job summary

Avamere Living is hiring a Licensed Practical Nurse (LPN) to join a memory care and assisted living team in Bend, Oregon. You will administer medications, monitor residents, and collaborate with an RN to ensure high-quality care for seniors.

The role requires an active LPN license in Oregon and experience in geriatric or memory care settings, with leadership responsibilities and strong communication skills. On-site position with comprehensive benefits provided.

Qualifications

• Active LPN license in Oregon
• Experience in memory care or geriatric settings preferred
• Strong leadership and communication skills
• Familiarity with CMS/state regulations for ALF/IL/MC facilities
• Experience documenting in electronic medical records
• CPR/BLS certification maintained during employment

Responsibilities

• Administer and document medications and treatments.
• Monitor residents' health and vital signs; perform routine assessments.
• Tailor care to residents with dementia and Alzheimer’s.
• Execute and adjust ISPs under RN direction.
• Supervise CNAs and care staff; provide guidance.
• Communicate with families and physicians about resident status.
