Job summary

Avamere in Bend, Oregon, is seeking a Licensed Practical Nurse (LPN) for a full-time role at Arete at Desert Sage Retirement. The LPN will oversee medications, monitor resident health, and support memory care residents under RN supervision.

The position includes supervising CNAs, coordinating with families and physicians, and executing ISPs while ensuring compliance with CMS/state regulations. Strong leadership and geriatric care passion are essential.

Qualifications

• Must possess a nursing degree and an active LPN license in this state.
• Experience in assisted living, memory care or geriatric settings preferred.
• Knowledge of CMS/state regulations and compliance requirements.
• Experience with electronic medical records documentation.
• Strong leadership, communication, and organizational skills.
• CPR/BLS certification required and maintained.

Responsibilities

• Medication administration with safe handling and documentation.
• Monitor residents' health, vital signs, and routine assessments.
• Provide memory care and behavioral support for dementia residents.
• Execute and adjust ISPs under supervision of RN/wellness director.
• Supervise CNAs and care staff; mentor team members.
• Communicate with families and doctors regarding orders and updates.
• Maintain resident assessments and care plans; ensure confidentiality.
• Provide training on dementia care and emergency procedures.
