Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living is seeking an experienced Licensed Practical Nurse (LPN) for Arete at Desert Sage Retirement ALF in Bend, OR. The LPN will provide direct nursing care, manage medications, and support residents’ cognitive and physical needs under RN supervision.

The role emphasizes leadership of frontline staff, collaboration with physicians and families, and adherence to CMS/state regulations in a memory-care setting.

Qualifications

• Must possess a nursing degree from an accredited college.
• Must possess a current, active LPN license in this state.
• Experience in assisted living or elder care is preferred.
• Knowledge of CMS and State Regulations for patient care in ALF/IL/MC facilities.
• Experience in EMR documentation.
• Strong leadership and communication skills.
• CPR/BLS certification maintained during employment.
• Confidentiality of resident information required.

Responsibilities

• Administer medications, document treatments, and conduct regular narcotic counts.
• Monitor residents' health, vital signs, and cognitive changes.
• Provide dementia-focused care and redirection for memory-care residents.
• Follow ISPs and update care plans with RN oversight.
• Supervise CNAs and other staff during daily routines.
• Communicate with physicians and families about resident care.
