Avamere Skilled Advisors, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct medical care in a memory care and assisted living setting in Bend, OR. The role involves medication management, health assessments, and coordinating with RNs, doctors, and families to support residents.

You will supervise CNAs and care staff under RN supervision, ensure ISP care plans are implemented, and maintain regulatory compliance while delivering compassionate, resident-centered care.

Qualifications

• Must possess a nursing degree from an accredited college.
• Must possess a current, unencumbered, active LPN license in this state.
• Experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute or critical care settings.
• Strong leadership or mentorship and communication skills.
• Knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility.

Responsibilities

• Medication administration: Safely prepare, administer, and document medications and treatments.
• Health monitoring & assessments: Monitor vital signs and assess resident health; track changes.
• Memory care focus: Tailor care to residents with Alzheimer's or dementia and manage behaviors.
• Care plan execution: Follow ISP plans and adjust care as resident needs evolve.
• Staff supervision: Oversee CNAs and care staff, providing guidance.
• Family & doctor liaison: Communicate with physicians and families.
