Arete at Desert Sage Retirement in Bend, OR is seeking a Licensed Practical Nurse (LPN) to provide direct medical care, manage medications, and support residents' cognitive and physical needs under RN supervision. The LPN will oversee frontline caregivers, assess health, and communicate with families and doctors in a memory care and assisted living setting.

Qualifications

• Must possess a nursing degree from an accredited college.
• Must possess a current, active LPN license in Oregon.
• Experience in assisted living, Alzheimer’s/dementia, long-term care, geriatric rehab, sub-acute or critical care.
• Knowledge of CMS and State regulations regarding patient care.
• Experience in electronic medical records documentation.
• Strong leadership and communication skills.
• Maintain CPR/BLS during employment.
• Confidentiality and compassionate elder care.

Responsibilities

• Safely prepare, administer, document medications; conduct narcotic counts; ensure regulatory compliance.
• Assess residents on move-in; conduct routine health checks; monitor vital signs.
• Tailor care for Alzheimer's/dementia; provide redirection and cognitive support.
• Follow ISPs; adjust care as needs evolve.
• Supervise CNAs and caregivers; provide guidance.
• Communicate with physicians; provide updates to families.
• Conduct thorough resident assessments; update care plans.
• Educate staff on dementia practices, infection control, emergencies.
• Ensure daily routines are followed to support memory care residents.
• Provide emotional support and connect residents to community resources.
• Handle emergencies and evolving healthcare situations with critical thinking.
• Other duties assigned are appropriate.
