Trilogy Health Services, LLC is seeking a Memory Care Leader to oversee the Legacy Lane memory care program at our Lafayette campus. You will ensure dementia care practices are implemented and support residents with cognitive impairment, working closely with the executive team.

You will lead education on dementia care topics for campus staff and families, participate in resident-centered planning, and ensure compliant staffing for the memory care unit.

Formación

• 1–3 years of relevant experience preferred.
• Strong interpersonal and communication skills.
• Ability to lead dementia care initiatives and educate staff.

Responsabilidades

• Oversee daily operations of the Legacy Lane memory care program.
• Lead dementia care best-practices initiatives across the campus.
• Educate staff and families on dementia care topics as Dementia Champion.
• Ensure appropriate resident population per memory care move-in/move-out criteria.
• Develop personalized Service Plans for memory care residents and guide families.
• Conduct interviews for Legacy team members and monitor safety in the unit.
