Job summary

Trilogy Health Services, LLC is seeking a Memory Care Leader (RN) to oversee the Legacy Lane memory care program on our health campus. You will implement dementia care practices and support residents with cognitive impairment, guiding staff and families through the care continuum.

Lead dementia education initiatives, collaborate with leadership, and ensure staffing aligns with specialized dementia care needs across the campus.

Qualifications

• Must have and maintain a current, valid state RN license.
• Nursing degree from an accredited college or university.
• 1–3 years of relevant memory care experience preferred.

Responsibilities

• Oversee daily operations of the Legacy Lane memory care program.
• Serve as Dementia Champion; educate campus staff and families on dementia care topics.
• Manage resident population in memory care per move-in/move-out criteria.
