Job summary

The Memory Care Leader at Trilogy Health Services, located at Autumn Woods Health Campus in New Albany, oversees the Legacy Lane memory care program, ensuring dementia care practices are implemented and resident wellbeing is supported.

Responsibilities include leading staff education on dementia topics, guiding family members through care planning, and coordinating staffing to maintain appropriate resident populations on the memory care unit.

Qualifications

• Associate’s degree preferred.
• 1–3 years of relevant experience preferred.

Responsibilities

• Oversee daily operations of Legacy Lane memory care program.
• Lead initiatives to educate staff on dementia care topics.
• Support memory care move-in/move-out criteria and staffing plans.
• Develop personalized Service Plans for memory care residents.
• Ensure dementia-safe environment with regular rounds.
