Job summary

Trilogy Health Services, LLC in Pickerington, OH is seeking a Memory Care Leader (CNA) to oversee the Legacy Lane memory care neighborhood, ensuring dementia care practices are implemented and residents with cognitive impairment receive compassionate, individualized care.

You will educate campus staff and families about dementia topics, guide care planning through Resident First Meetings, and collaborate with leadership to maintain appropriate staffing and safety on the memory care unit.

Qualifications

• High school diploma or GED preferred.
• Current CNA certification required.
• 1–3 years of relevant experience preferred.

Responsibilities

• Oversee daily operations of the Legacy Lane memory care program.
• Educate staff and families on dementia care topics as the campus Dementia Champion.
• Maintain memory care resident population per move-in/move-out criteria.
• Develop personalized service plans for memory care residents and guide families.
• Ensure dementia-safe environment with daily rounds on the unit.
• Participate in interviews to staff the memory care team.
